PLEASE READ CAREFULLY. BY CHECKING “I AGREE” AND SUBMITTING THIS FORM, YOU ARE WAIVING CERTAIN LEGAL RIGHTS.

1. Assumption of Risk

By participating in the Make Jesus Known Christmas Festival—including but not limited to activities, prizes, and gifts such as inflatable games, foam axe throwing, basketball, Marlrin foam ball shooting, giant dart games, worship service, gingerbread house decoration, jumbo soccer darts, giant tic-tac-toe, skee-ball, ring toss, wooden toppling tower, inflatable football toss, cornhole, pool fishing, face painting, arts and crafts, slime, toys, party stuffers, candy, foam toys, foam gliders, electric scooters, iPads, crayons, party favors, scratch art, squishy toys, bouncing balls, coloring books, and consumption of food, snacks, and beverages—I acknowledge that participation involves inherent risks.

These risks include, but are not limited to: slips, trips, falls, collisions, equipment malfunction, allergic reactions, choking, injuries, illness, and property damage. I voluntarily assume all risks for myself and any minor children I bring to the event.

2. Release of Liability

By submitting this form, I release, waive, and discharge:

Make Jesus Known, including its directors, officers, pastors, employees, volunteers, representatives, agents, event partners, and property owners. That includes the entity and property owner of Highland Covenant Church located at 15022 NE Bel Red Rd, Bellevue, WA 98007.

from any and all claims, liabilities, demands, or causes of action arising from any injury, illness, disability, death, or property damage that may occur during or as a result of attending or participating in this event.

This release covers claims resulting from the ordinary negligence of Make Jesus Known or its volunteers.

3. Indemnification

I agree to indemnify and hold harmless Make Jesus Known and all volunteers and representatives from any loss, liability, or costs arising from my or my child’s participation in the event.

4. Medical Authorization

If a medical emergency occurs involving me or my minor child(ren), I authorize Make Jesus Known personnel or volunteers to obtain medical treatment deemed necessary. I accept full financial responsibility for any medical care provided or necessary during the event or in the future.

5. Photo/Video Consent

I understand and agree that photos, videos, and recordings may be taken during the event.
By submitting this form, I grant Make Jesus Known and any other 3rd parties authorized by Make Jesus Known the unrestricted right to use, reproduce, and publish any such media featuring me or my minor child(ren) for promotional, ministry, social media, fundraising, and organizational purposes, without compensation.

6. Parent/Guardian Consent

If I bring minor children or a minor child to the event, I confirm that I am their parent or legal guardian and consent to their participation under the terms of this waiver. I understand I am responsible for supervising any minors I bring during the entirety of the event and my time on the premises.

7. Electronic Agreement

By checking “I HAVE READ AND AGREE TO THE TERMS OF THE CHRISTMAS FEST RELEASE & WAIVER” and submitting the form on the CHRISTMAS FESTIVAL CHECK-IN page:

  • I confirm I have read and understood this Release & Waiver.

  • I agree to all terms voluntarily.

  • I understand that my electronic submission is legally binding and has the same effect as a handwritten signature.
